File an application for creating a large foster family and place the child
You need to file an application for creating a large foster family, detailing any debt liabilities, with a duplicate of the package of documents.
Submit the application to the authority that creates the large foster family (Oblast, district state administration in Kyiv and Sevastopol, or the executive body of the city council).
The Oblast, district state administration in Kyiv and Sevastopol or the executive body of the city council must adopt a decision on creating a large foster family based on the application, the opinion of the Children’s Services and recommendations given by the social service centre.
Outcome of Step:
The Oblast, district state administration in Kyiv and Sevastopol or the executive body of the city council adopts a decision on creating a large foster family. The decision is documented as an ordinance.
Conclude the agreement on organising the activities of the large foster family
After the decision to create a large foster family, the caregivers and the decision-making authority conclude the agreement on organising the large foster family.

Outcome of Step:
The caregivers and the decision-making authority conclude the agreement on the large foster family which stipulates the list of obligations and the scope of responsibility of the caregiving parents in connection with the child, as well as obligations of the decision-making authority, to ensure the smooth operation of the large foster family.
